ALUMNI SERVICES

The following services are available to alumni of the School of Medicine and Biomedical Sciences:

Transcripts
Copy of Dean's Letter
MD Verification Letter
State Licensure Verification
Diploma Certification* { instructions }

All requests must include the following information:

• Name
• Social Security Number
• Year of graduation
• Phone number
• Third party address(es) who should receive the requested documents
• A $5.00 fee for transcripts.
  Requests for this service can not be made via e-mail.
  Payment should be made in the form of a check or money order made payable to: University at Buffalo. Requests made in person can be paid in cash.
  There are no fees for the other services listed above.

Special Services:

* Certification of Copy of Diploma: You must send us an 8.5''x11''copy of your original diploma. The OME will verify that it is a true copy of the original and will affix the UB seal on the copy.

* Replacement Diplomas: Contact OME at (716) 829-2802 for a replacement application. A diploma can only be issued for a name change or if evidence shows that the original diploma has been irretrievably lost or destroyed. The fee is $20.00.
