Ambulatory Care

At the Buffalo General Hospital, the fellows participate in a weekly outpatient hematology clinic as well as seeing private hematology consults on a daily basis as outpatients in medicine’s ambulatory care facility. At the Erie County Medical Center, the fellows participate in a weekly hematology clinic. At this site the fellows are also involved in ambulatory care activities of the Hemophilia Center. They also participate in transfusion medicine programs at the Erie County Medical Center. At the Buffalo VA WNY Healthcare System, the fellows participate in two clinics, one a hematology clinic and the second a clinic of patients with oncology and hematologic malignancies. At this site, they are also heavily involved in ambulatory chemotherapy in the VA Cancer Center. The hematology fellow would see approximately 300 new outpatients in a given year and follow 100-150 patients chronically in a year. Attending supervision is present but the hematology fellows serve as the primary consultant or provider for these patients. One clinic per week per resident is at the same site for the duration of the program. For the additional approximately 2 clinics per week, the hematology fellow is present for four months and thus is able to provide continuing care for many patients. Adequate attending staff is present at all ambulatory care sites and are directly involved in discussion and participation in care of the patients. Each patient is discussed at the end of the clinics in a group session and aspects of care and education are raised during these approximately 30-45 minute informal conferences.
