An
Approval Form must be completed prior to submission of each Sponsored
Programs proposal or the acceptance of any award.
1. Provide the information requested
about the project director, (PD), i.e. the single person who will
accept responsibility for managing the project.
2. Check the box that most
closely describes the nature of the project. Training programs provide
support for graduate and/or undergraduate students working toward
a degree or for post-doctoral students receiving advanced training.
Educational support programs provide funds to develop or strengthen
programs but do not provide direct support for trainees.
3. Check the appropriate boxes
and when applicable, provide the account number of the active project
that the proposed work will continue, renew or supplement.
4. Provide the complete title
of the project exactly as it appears on the proposal face page.
Please limit title to 120 characters (including spaces) whenever
possible.
5. Check the appropriate boxes.
Proposals seeking support from private, non-profit or for-profit
sponsors will ordinarily be submitted through the UB Foundation
Services (UBFS) unless the PD indicates that administration of the
project by the Research Foundation is preferred. Proposals to federal
or state agencies, or to foreign governments, are always submitted
through the Research Foundation.
6. Check the appropriate box.
Proposals that seek support for the same technical program and differ
only in format and/or budget are "substantially the same".
7. Check the appropriate box.
If the proposal is in response to a solicitation (i.e. a program
announcement), request for application (RFA), or a request for proposal
(RFP) which was not provided to you by the Office of Sponsored Programs
Administration (OSPA), please attach a copy of the solicitation
so that OSPA staff can ensure that the proposal meets all of the
solicitation requirements. If the proposal is unsolicited but being
submitted to a funding source from which the PD obtained application
materials directly please check with OSPA to be sure this information
is on file. If it is not, a copy should be attached to the proposal.
8-12. Provide the information
requested.
13. Provide the specific address,
including phone number, to which the proposal is to be submitted.
14. If the project is to be conducted on campus (affiliated hospitals
other than the Veteran's Affairs Medical Center are considered on-campus
locations), indicate the building(s) in which the project will be
conducted. If all or part of the project is to be conducted off-campus,
indicate the on- and off- campus locations and the portion of the
project to be conducted on-campus.
15-16. Provide information requested.
The forms used to request Human Subjects or Institutional Animal
Care and Use Committee approval may be obtained from OSPA.
17. If yes, contact OSP A to
discuss FDA procedures and requirements.
18. All proposals that involve
the use of recombinant DNA must be reviewed and approved by the
Biosafety Committee. Provide information requested.
19. If the proposal includes
less than the appropriate full indirect cost recovery, a waiver
of indirect costs must be requested and approved prior to submission
of the proposal. Request for waiver should be made in accordance
with the University Policy on Recovery of Sponsored Program Costs
and submitted to the Assistant Vice President for Sponsored Programs
Administration. Waiver requests will be submitted by OSPA to the
Vice President for Research for approval.
20. If any portion of the project
will be subcontracted, the subcontractor must be named. In addition,
the scope of work, an approved budget and letter of intent from
an authorized official of the subcontracting organization must be
included with the proposal.
21.
a.
If University resources (e.g. faculty effort, student time, equipment,
etc.) are being committed to the proposed project, indicate whether
the contribution is required by the sponsor.
b.
If "a." is checked "yes", indicate the total value of the University
resources being committed to the project.
c.
If "a." is checked "yes", the University official responsible
for managing the resources committed to the project must initial
this line.
22.
If the sponsor requires that a third party, i.e. one other than
the University or the sponsor, contribute to the costs of the project,
provide the information requested. A written commitment from a third
party to provide matching funds for the project should be included
in the proposal and must be received by OSPA prior to acceptance
of an award.
23. If the proposal will result
in an award which obligates the University to support personnel,
equipment or facilities after completion of the award, indicate
the duration of the continuing obligation, the estimated cost per
year of the continuing obligation, and the source of funds to meet
the continuing obligation.
24. If the project will require
either space or facilities not committed to the project at the time
the proposal is submitted, or renovation of existing space to meet
the needs of the proposed project, use a separate sheet to detail
the additional requirements and estimate any costs. If such costs
are not included in the project budget, explain how they will be
met.
25. If the establishment of
a new academic program is proposed, review and approval of the proposal
by the Provost is required prior to submission.
26. If the proposed project
will involve provision of information proprietary to the sponsor,
i.e. information owned by the sponsor upon which the sponsor imposes
restrictions in access and/or distribution, discussion with OSPA
is required about the conditions under which the University can
accept such information is required.
27. Provide the requested information.
28. This item is used to assign
credit for the proposal and any subsequent award and expenditures
and to acknowledge co-investigators/co-project directors. It must
be completed for all proposals. The head of the department or unit
in which the project director holds a primary appointment must approve
all proposals, even when no credit is assigned to that department,
e.g. if 100% of the credit for a project is assigned to a Center.
a.
To assign credit for the entire project to the project director
in an academic department, enter the department in 28c. and 100%
in 28d.
b.
To assign some or all credit for the proposal and any resulting
award and expenditures to other than department or unit in which
the project director holds a primary appointment: enter the departments/units
to which credit is to be assigned in 28c and the share to be assigned
to each on the corresponding line of 28d. The head of each department
or unit listed in item 28 must approve the proposal.
c. To acknowledge co-PD's
(co-investigators): enter each co-PD in 28a and her/his department
or unit in 28c beginning on the second line. Each co-PD must initial
the corresponding line in 28c and the head of the unit in which
the co-PD holds a primary appointment must approve the proposal.
d.
To assign a portion of the credit for the proposal and any resulting
award and expenditures to co-PD's departments or unit: proceed
as in c. above and enter the share to be assigned to each co-PD's
department in 28d in integer increments.
Note:
Signature of the PD in Item 29 and initials of Co-PDs in Item 28
certify that s/he (they) are not, to the best of knowledge, presently
debarred, suspended, proposed for debarment, declared ineligible,
or voluntarily excluded from transactions by any Federal department
or agency and that s/he (they) have on file with the cognizant dean(s)
current and complete Form SUNY -2-UB financial disclosure.
29. The project director must sign
and date the Approval Request.
30-33. Obtain approvals as specified.
The Approval Request must be approved by the head of the department
in which the project director holds a primary appointment and by
the head of each Department/Unit named in item 28. Each dean who
has cognizance of one or more departments listed in item 28 must
approve the proposal. Direction of a sponsored program by a member
of the professional staff must be approved by the Provost or Vice
President to whom the professional staff member's unit reports.
