Gold Choice PCMP IIA is a partially capitated Medicaid Managed Care Program utilizing the Physician Case Management Model for ensuring access to Primary Care and specialty Medical Services.  The contract with the Erie County Department of Mental Health is jointly funded with New York State Offices of Alcoholism and Substance Abuse Services and Mental Health funding streams.

The program currently has 4600 enrollees referred by over thirty (30) Mental Health and/or Substance Abuse Services agencies located in Erie County.  Such providers consistently see the program as the Medicaid Managed Care Program of choice for individuals with a mental health and or chemical dependency diagnosis. 

The contract with the Erie county Department of Mental Health provides administrative supports and specialized case management services that ensure that at risk populations have access to specialized medical services and that such services are coordinated with their mental disability services.
